
The community lung health programs are available to individuals living with asthma, bronchiectasis, chronic obstructive pulmonary disease (COPD), and interstitial lung disease (ILD). Programs provide the education and support needed to help people manage these chronic lung diseases. Services include assessment, diagnosis (in most areas), education, medical management, and pulmonary rehabilitation for COPD (in most areas),
Services vary by location, click here to find a list of program locations and services. Services are available in the counties of Lanark, Leeds and Grenville, Renfrew, Prescott-Russell, Stormont, Dundas and Glengarry, as well as in Cornwall and Ottawa.

The Community Lung Health Programs offering the services:
Program staff include Certified Respiratory Educators (CRE), such as a Nurse Practitioners (NP) and/or Registered Respiratory Therapists. The community lung health program will contact your patient to set up an appointment and will report back to you regarding the status of your client’s referral.
Facts on community lung health services:
- Clients who are referred will receive individualized and/or group sessions. The educators assess client needs and use a combination of clinical knowledge and health coaching to facilitate learning and enhance self-management skills.
- Pulmonary rehabilitation (PR) and Maintenance PR is available at some locations.
- Education and individualized care plans for achieving self-management.
- Ongoing assessment and treatment of frequent exacerbations or ER visits.
